UCC Registration Requirements:

• Proof of residence and a picture ID
• Insurance cards (We accept most Insurance plans)
• Referral or Authorization (when applicable)
• Cash, Check, Master Card, VISA and Discover are accepted for payment at the time of service

Specialty Clinic Registration Requirements:

• Valid appointment and/or referral form
• Proof of residence and a picture ID
• Insurance cards (We accept most Insurance plans)

WHAT DOES IT COST?

For those without insurance, patient fees are based on the patient’s ability to pay using specific resident criteria and the current Federal Poverty Guidelines.

You may be eligible for reduced or no payment rates depending upon:

• your verified residence
• your referral source (when applicable)
• proof of gross household income
• verification of the number of people being supported by this income
