Hours of Operation:

Monday – Friday, 8:00 a.m. – 4:30 p.m. 

HIS department is closed for the following holidays: New Year’s Day, Martin Luther King Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Friday after Thanksgiving and Christmas Day

Fees for Copying Medical Records: 

$21.36 processing fee, $.50 per page, plus postage if records are mailed.
